A sweeping new analysis of more than 1.9 million people has revealed that individuals diagnosed with attention-deficit/hyperactivity disorder face significantly higher odds of suffering from gastrointestinal problems, including nearly double the risk of constipation and quadruple the risk of losing bowel control.
Researchers at the
University of Warwick published their findings Sept. 10 in the
Journal of Attention Disorders, pooling data from 23 previous studies conducted across the United States, Sweden, Germany and South Korea. The review, which examined people ages 2 to 65, marks the first systematic analysis dedicated specifically to digestive symptoms in people with ADHD. While the study confirms an association rather than proving causation, the pattern was strong enough that researchers are urging clinicians to pay closer attention to gut health when treating patients with the condition.
Digestive symptoms turn up across the board
People with ADHD were found to experience gastrointestinal symptoms 48% more often than those without the condition, the meta-analysis found. The gap widened further for certain conditions: constipation occurred at nearly double the rate, irritable bowel syndrome at a 58% higher rate, and encopresis — the medical term for loss of bowel control — at more than four times the rate. Indigestion also showed up more often among those with ADHD.
Co-lead author Sarah Blake, a medical student at
Warwick Medical School, said these symptoms can have "a real impact on daily life, but they could easily be overlooked if the focus is on ADHD itself."
What might explain the connection
Researchers outlined several potential drivers of the link, though they emphasized they cannot yet determine whether ADHD directly causes digestive problems. ADHD-related traits such as impulsivity and irregular eating patterns may affect digestion, and medications commonly prescribed for ADHD are known to produce gastrointestinal side effects such as constipation.
A third theory points to the gut itself. Earlier research has found that people with ADHD tend to have different populations of gut bacteria than people without the condition, and scientists suspect that imbalance could interfere with signals sent along the vagus nerve, the primary channel linking the digestive system to the brain. The study builds on a growing body of evidence pointing to a "gut-brain axis" that connects digestive health to neurodevelopmental conditions more broadly, although previous research had not examined ADHD-specific gut symptoms as closely.
Doctors should start asking patients about GI symptoms
Senior researcher Dr. Saran Shantikumar, an associate clinical professor at
Warwick Medical School, said the findings suggest doctors caring for people with ADHD should consider asking about gastrointestinal symptoms, particularly if those symptoms might affect quality of life. For patients with more complex or severe digestive issues, researchers recommended involving a dietitian or gastroenterologist in their care.
ADHD is estimated to affect roughly 5% of children and 3-4% of adults worldwide, and diagnosis rates have climbed steadily over the past 40 years, and there has also been a rise in prescriptions for stimulant medications long known to carry constipation and other gut-related side effects. Shantikumar noted that while the study cannot yet prove ADHD causes gut problems, "the pattern is striking enough to warrant attention."
For families already dealing with an ADHD diagnosis, the findings are a reminder that medication trade-offs deserve the same scrutiny as the condition itself — and that patients are entitled to know exactly what side effects they may be signing up for before starting a prescription.
